Annual forms are required for every student, every school year.
Our records show that some families have not yet completed their student’s annual forms. These forms were due September 1 and need to be completed as soon as possible.
Every year, every form
Annual forms must be reviewed and submitted every school year, even if nothing has changed. If your information is still correct, you still need to review the form, indicate that no changes are needed, and submit it.
Completing the forms helps make sure we have the most current information we need for your student and provides information required for school funding and reporting.
How to complete your forms?
Please log in to PowerSchool and complete all required forms.
Once you log in:
Select
Forms
on the left.
Open your student’s
School Form Listing
.
Review
all
required annual forms.
Make any needed updates, or indicate that no changes are needed.
Sign, date, and submit
each required form.
